VALA Membership Application – Provider

Provider Member – Any company or organization, proprietary or nonprofit, which owns, manages or operates an assisted living facility or similar type of facility. This includes facilities under construction.
Dues are calculated on resident capacity per facility. Providers with multiple facilities must complete an application  for each facility. 

• Annual Membership Directory (With Quarterly Updates)
• A voice in Virginia's legislature with regular legislative updates
• Advocacy with Virginia's regulatory offices (i.e. DSS, DHP, DMAS, VDH)
• Continuing Education Opportunities
• Quarterly Newsletter with information pertinent to ALFs
• Regular communication on important issues
• Participation opportunities on Boards and Committees
• Discounts on VALA's Annual Statewide Conferences
• Membership Referral Services
• Networking Opportunities

Annual Membership Fee is determined by facility size. Annual VALA Dues Calculation:

A.L. Bed Capacity  _________ x  $15.00    =   $________________  ($250.00 minimum)

Industry Partner - This class of membership is for vendors or organizations that provide a service to or have an interest the assisted living industry in Virginia. Industry Partnership Membership Benefits include:

• Annual Membership Directory (With Quarterly Updates)
• Quarterly Newsletter
• Discounts on Annual Statewide Conference
• Membership Referral Services
• Networking Opportunities

(Membership is $500 per year.)
